St. Jude Medical Center offers special programs through our Senior Services, which address the unique needs of older adults and their families in North Orange County. Our mission is to help low income seniors stay healthy, independent and active by providing comprehensive programs and access to services. Senior Services provide transportation, dedicated post-treatment counselors, in-home safety, depression avoidance and treatment resources, grief counseling. Our Medicare Insurance Plan Specialist can give free information on all your questions.

Determining your wishes for medical care at the end of your life is called advance care planning. It's a way for you, your loved ones and your physician to discuss the kinds of care you want and don't want at that time. You can also specify the care you would want if you become unable to speak or make decisions for yourself, due to a coma or other medical condition. When you write down your wishes, this kind of plan is called an advance directive. We encourage all patients to complete an Advanced Care Directive before being admitted and bringing it with you so we may honor your wishes for care.

The best time to start the conversation about the kind of care you’d want if you were in an accident or became seriously ill is now. Let us help you think about the care you’d want, talk to your loved ones about your decisions, choose your advocate and complete an advance directive.

Supported by St. Jude Medical Center, The Caregiver Resource Center (CRC) is the only Orange County organization that partners with families who are caring for an adult family member. The CRC is specifically focused on the needs of caregivers who are coping with a loved one's chronic condition and has supported thousands of caregivers since 1988. Free Advance Care Planning classes held Thursdays, 1:30 - 3 p.m. at local St. Jude Heritage Medical Offices and in the community. Free notary services, resources, information and assistance. Caring Neighbors/Friendly Visitor Program

Free volunteer service to help low-income seniors increase socialization, reduce isolation and the heartbreak of loneliness. Regular home visits, friendly phone calls, errands, light housekeeping, cheering conversation and walks around-the-block if they’d like (program suspended during COVID). Cheer-A-Senior Program
  • Weekly phone check-in calls, grocery/medication pick-up, chair exercises, conversations and relationship building

Depression Solutions in Late Life
  • Free in-home depression assessment, care management and short term counseling with a licensed clinician for low-income seniors, and linkage to community resources.
  • Free senior depression support group. Learn strategies to combat depression and anxiety. Develop tools to strengthen resilience.

Fall Risk Reduction
  • Free in-home safety assessments for older adults
  • Referrals for needed resources, including home repairs, handyman services, personal organizers, placement specialists, etc.

Healing Hearts after Loss Bereavement Group

This free grief recovery support group is for anyone who is working through the pain of loss. Personal and warm, wherever you’re at in the grieving process. Attend the group as long as you’d like, whenever you’d like – you’re always welcome.

  • Deal with the tasks of mourning, celebrate and remember a life well loved

Free one-hour class every 4th Wednesday, 7-8 p.m. at 130 W. Bastanchury Rd, Fullerton. Also available as a Zoom class. Get information about:

Parkinson's Disease education and support group

This group, which meets on the first Thursday of every month from 3-4 p.m., provides education, resources, information and support to patients and families alike.  During meeting times, click here to join the Parkinson's disease support group zoom; use the Meeting ID 825 2640 1789. Stroke Survivors Support and Education Group

This is a free group for stroke survivors, caregivers and family members. Participants meet in person on the first and third Monday of the month from 1:30 to 3 p.m. Registration is not required, and walk ins are welcome. For more information, send us an emailor call 877-459-3627.

Technology Training – Digital Literacy Classes, WiFi and Tech Support

St. Jude provides resources for transportation services to patients who are unable to drive to doctor appointments and therapy sessions.
