Patients who no longer need acute or intensive care, but are not yet ready to be discharged, are cared for in our Transitional Care Center. Our focus is on helping each patient achieve their highest level of functioning so they can return to the community with more strength and greater independence. Our multi-disciplinary approach involves the expertise of physical therapists, occupational therapists, social workers, rehabilitation medicine physicians, pain management specialists, recreational therapists, speech pathologists, respiratory therapists and others. While our focus is different from the acute units of the hospital, our patients access the same wide range of specialists and services. Therapy is individualized to meet the unique physical and emotional needs of each patient. We emphasize patient and family education, and offer innovative services such as painting, cooking and art classes to help patients re-learn their favorite hobbies. The quality of care provided by our Transitional Care Center is well documented in the recent accreditation survey by the Department of Health Services (DHS). Five DHS surveyors spent three days reviewing every aspect of our care - from patient safety to quality of patient education - and awarded St. Jude one of the state's highest scores. |
