The Medical Director is a physician who actively participates in the patient’s pain and symptom management. The Medical Director offers consultation to team members and the patient’s attending physician.

The Attending Physician usually makes the referral to hospice. This can be the patient’s family doctor or a specialist. The Attending Physician continues to be an essential part of the patient’s care planning. Our team works closely with the Attending Physician to obtain orders and update him/her regarding the patient’s status.

A Registered Nurse case manager is assigned to every patient. We value continuity of care and we make every effort to have one primary nurse involved in the patient’s care. The nurse offers warm, compassionate and supportive patient care. The nurse's role is to enhance the patient’s quality of life by managing pain and symptoms, provide education, and allow meaningful experiences with family and friends. A Certified Home Health Aide is available on an intermittent basis to provide bathing and personal care. The Aide works along side of the nurse to enhance quality of life and dignity for the patient.

A Clinical Social Worker is assigned to each patient and family to provide psychosocial support, counseling and advocacy to help the patient and family adjust to the illness and prognosis. The Social Worker’s expertise is to access resources for end of life care planning.

A Hospice Chaplain is available to every patient and family member who may have spiritual issues or desires to connect with religious traditions and rituals. The chaplain helps patients and families find meaning in the significant events of their lives. The chaplain completes our circle of care by providing support to patients of all faiths and denominations.

A Hospice Volunteer is a dedicated and trained individual who joins the hospice team in providing comfort and support to the patient and family. The volunteer offers the patient and family companionship, respite and assistance with activities of daily living. The volunteer is an essential part of the team providing special nurturing and attention to patients and families. A Registered Dietician offers consultation to the hospice team regarding diet and nutrition. The RD is also available to provide education and support to the patient and family regarding diet and nutrition.

A Music Therapist offers music therapy services to patients and families in an atmosphere of trust and acceptance. Our Music Therapists are nationally certified by the American Association for Music Therapy.

A Massage Therapist provides massage therapy services to hospice patients under the order of their Attending Physician. Adjunct therapies have been found to be quite successful in the relief and management of pain and other symptoms.

A Bereavement Coordinator directs our bereavement staff as they provide bereavement support and education to our hospice families for one year following the death of their loved one.

Physical, Occupational and Speech Therapists provide consultation, assessment and instruction on a short-term basis.
