Medical Insurance All FT/PT employees (and their spouses / dependents) HMO and POS plans available through a major health insurance company. 1st day of the month after 30 days of service
Prescription Coverage Included with Medical Insurance coverage Prescription drug coverage with $10 copay for generic, $25 for name brand medications and the 3rd tier for non-preferred brand medication at a co-pay of 40 percent of the drug cost ($30 minimum / $60 maximum co-pay.) 1st day of the month after 30 days of service
Dental All FT/PT employees (and their spouses / dependents) Choose from two major dental insurance providers. Each offers diagnostic and preventative care coverage, restorative care, and orthodontia. 1st day of the month after 30 days of service
Vision All FT/PT employees (and their spouses / dependents) Coverage allows for periodic eye exams, lenses and frames with appropriate co-pay. 1st day of the month after 30 days of service
Term Life, Accidental Death and Dismemberment All FT/PT employees Basic term life insurance offered at 1 times annual salary. Supplemental insurance available at fixed amounts up to $1,000,000. 1st day of the month after 30 days of service
Dependent Life Insurance All FT/PT employees Provide coverage for a child and/or a spouse. 1st day of the month after 30 days of service
Long Term Disability All FT/PT employees Benefit-eligible employees receive a base plan that provides for 50 percent of base pay with a 180-day waiting period. Employees have an option to upgrade coverage. Plan is integrated with other coverage, i.e. SDI or WC. 1st day of the month after 30 days of service
Tax-Free Dollar Plan (Flexible Spending Account) All FT/PT employees A voluntary plan that allows you to pay your share of health care and dependent day care expenses on a pre-tax basis. The amount that you estimate to cover expenses for a calendar year is deducted from your paycheck before taxes. 1st day of the month after 30 days of service
Retirement Plan All employees 401K Contributory Plan with company match plus a company sponsored 401(A) plan providing between 3-10 percent based on eligible waiting period. See plan guidelines
Paid Time Off (PTO) All FT/PT employees PTO is available for you to use for vacation, holiday, short-term illness or other personal illness. Accrual is based on actual hours worked up to a maximum of 80 worked hours per pay period. PTO hourly accrual is based on years of service in a benefited position. Upon employment
Extended Sick Time All FT/PT employees These hours can be accessed after being ill for 24 scheduled work hours for a single illness. Upon employment
Group Legal Plan All FT/PT employees A voluntary plan that provides you with a variety of legal services. 1st of the month after 30 days of service
Long Term Care All FT/PT employees You will have the opportunity to purchase long-term care for yourself, your spouse, and other members in your family. 1st of the month after 30 days of service
Merit Increase Employees who meet eligibility requirements A merit increase based on a performance evaluation. Annually
Tuition Reimbursement All FT/PT employees Provides reimbursement for job-related course work. After qualification period
Credit Union All employees and members of their immediate family Offers a variety of services, including checking and savings programs, vehicle and personal loans, credit cards at competitive interest rates, direct deposit, payroll deductions, and US Savings Bonds. Upon employment
Premium Holiday Pay All FT/PT employees Time and a half hourly rate paid for hours worked on selected holidays. Upon employment
Shift Differential All FT/PT employees Hourly differential paid to employees who work evening and/or night shifts. Upon employment
Bereavement Leave All FT/PT employees Bereavement pay is granted in the event of the death of an immediate family member. Upon completion of introductory period
